Registration: Register online at www.kaduceusinc.com, by clicking on the link in the upper right hand corner. To register by mail, print the registration form, complete and mail to Kaduceus at the address on the form.
Registration Fee: $399 with credit card, cashier’s check or money order. Payment must be received 5 days prior to start of the class. A deposit of $199 will hold your place in class, until full payment is received 5 days prior to start of class.
Refund Policy: No refunds. Payment may be applied to future classes provided Kaduceus is notified in writing
(e-mail accepted) seven days prior to the start of the class.
Cancellation Notice: Kaduceus may cancel a class due to low enrollment. Twelve student enrolled minimum. If Kaduceus does not have 12 students enrolled 5 days prior to start of class, students will be notified by e-mail and you will automatically be enrolled in the next class.
Bring to Class: A Calculator, pencil, paper and your Confirmation Letter. Your Training Manual will be given to you the first day of the class.
Passing the Exam: To pass, a score of 75% or higher is required on the written exam and successful demonstration of the proper aseptic techniques in preparing compounded sterile products.
When: The class is scheduled for 2 consecutive weekends - Saturday & Sunday

(Our address is not on GPS or Google Maps so use 21017 Kuykendahl Rd., Spring, TX 77379 as a location address.) From Interstate 45 exit 70A and drive west on FM 2920. Drive 4.8 miles to the intersection of FM 2920 and Kuykendahl. Make a left on Kuykendahl. Stay in the right lane and make a right turn on Augusta Springs (just past Compass Bank). Make another right at the STOP sign onto Springbrook Plaza Drive. We are located in the two-story office building, directly behind the KFC and Compass Bank.

Upon successful completion of Kaduceus IV Certification Program, the student will have completed 20 hours of home study + 20 hours of instructor lead study and labs. These 40 hours meets the requirements for 40 contact hours (4.0 CEUs) of credit required by the Accreditation Council for Pharmacy Education.
